If you have email contact information for your DAF provider, you may send a PDF copy of your DAF letter or you may copy and paste the text into an email. You will need to enter the amount of your recommended grant and your DAF account number on the letter before mailing. How to make a gift by beneficiary designation
To leave a gift to Renown Health Foundation by beneficiary designation, you will need to contact your retirement plan custodian or administrator, life insurance company, or financial institution and complete the beneficiary designation form or "payable on death" form provided. Follow the instructions provided on how to return the form and memorialize your beneficiary designation.
